Mark does not bill to insurance directly; however, a statement or super bill can be provided to you that may be submitted to your insurance provider. In order to be reimbursed by your insurance company for psychotherapy sessions, you must have out-of-network benefits.
You can learn more by calling the behavioral health number on the back of your insurance card and asking these questions:
Do I have coverage for out-of-network providers?
What is the deductible I have to meet before my insurance starts reimbursing me for out-of-network providers?
Will I be reimbursed for 100% of the cost of my sessions or just a percentage of the cost?
-
A superbill is a receipt for your session that includes all of the information your insurance company will need to reimburse your session – info about your therapist, about the session, and about your diagnosis.
-
Mark typically works with adults, 21 years and older.
-
Any sessions that are not cancelled within 24 hours of the scheduled appointment will be charged the full session rate.
